What happens to my data?

The General Data Protection Regulation (GDPR) governs how we handle your confidential information held as part of your GP medical record. Up until the age of 16 in most cases, we can speak to your parent or carer on your behalf. Once you turn 16 the rules around this change and we can no longer speak to your parent or carer without your explicit consent.

Mobile telephone numbers and email addresses

Any mobile telephone numbers and email addresses currently held on your record will be removed. You should receive your own electronic communications (such as text reminders) going forward. 

To give us your own mobile telephone number and email address, please send us an email (from your own email address) with your full name, date of birth and updated contact details.

Online access

If someone else has online access to your medical record, this will now be removed. The easiest way to access your medical record online, is to download the NHS APP and follow the instructions to sign up. You can use the NHS APP to view your medical record, book appointments and order repeat medications.

If you still want your parent to have online access on your behalf, please complete and return the Proxy Online Access form.

If you would like to nominate a family member, carer or someone else to speak with the surgery on your behalf or to have access to your medical record:

You will need to complete the “Application for Third-Party Access to Healthcare Information” form – (This is a different form to the online access form).

The form will need to be signed and returned to the reception desk. If you want to send the form back electronically, please send it to [email protected] for the attention of Deepa Kapila. Someone from the practice will call you on the telephone to confirm it was you who completed and returned the form.

What to do if you are unwell

If you are unwell and you need medical advice you can:

  1. Log in to the NHS APP and book an appointment
  2. Fill in an online “PATCHS” form on our practice website. We will then have a look at the form and book you with the right person.
  3. Call the reception desk on 020 8630 1036

Please note that our appointments are released at 8:30am, so it is best to log on the APP or call the reception desk at that time.
